A North Carolina day care worker has been arrested and charged with involuntary manslaughter following the tragic death of a 1-year-old girl during her first day at the facility. Authorities say 29-year-old Alexandra Coffey was taken into custody in connection with the death of a toddler named Maddy at Creative Beginnings of Lenoir, where Coffey was employed.
According to WBTV, Coffey is being held at the Caldwell County Detention Center on a $500,000 secured bond. First responders were called to the day care around 2:45 p.m. on Monday after staff reported that Maddy was found unresponsive. CPR was administered in an effort to revive the child, but she was later pronounced dead at a nearby hospital.

Police suspect Maddy may have suffered cardiac arrest, but an official autopsy has been ordered to determine the exact cause of death. In a heartbreaking interview with WBTV, Maddy’s mother said she had dropped her daughter off at Creative Beginnings for the very first time on Monday morning. She later received a devastating call informing her that Maddy had not woken up from her afternoon nap.
Following the incident, the North Carolina Department of Health and Human Services took swift action, suspending the facility’s license. According to the emergency order, the suspension was based on “findings that the public health, safety, or welfare requires emergency action.” The day care was ordered to cease operations by the end of the day Wednesday.
In addition, the state halted all subsidized child care payments to the facility, further signaling the seriousness of the situation. While the investigation is ongoing, police said Creative Beginnings is fully cooperating with law enforcement. In a message shared on social media, the daycare expressed sorrow over the tragedy:
